Your first appointment
Arrive 15 minutes early. Bring these four things:
Completed patient forms
Print the packet and fill it out in pen before your visit.
Photo identification
A driver’s license or other government-issued ID.
Your insurance card
Bring your current card.
Previous records, if applicable
Bring records, chest X-rays, or CT scans on disk from your previous physician.
Manage your care
Patient portal
View results and health records, message your care team, request refills, manage appointments, and review statements.

Prescription refills
Request through your patient portal or contact your pharmacy. Allow 48–72 hours for processing.
Reschedule or cancel
Please give at least 24 hours’ notice. Phone hours are Monday–Friday, 9 AM–4 PM.

Common questions
- Which insurance plans do you accept?
- We accept Medicare and most major commercial plans. Check with our billing team or your portal to verify your specific network status. See accepted insurance
- Do you offer telehealth?
- All visits are in person at our Clearwater office. We do not offer telehealth, telemedicine, or video visits.
How our practice uses AI
Some providers use secure, HIPAA-compliant AI tools for clinical documentation and administrative tasks. Your provider reviews and verifies all documentation and remains responsible for medical decisions.
